Your old silver fillings are intact. A dentist suggested replacing them with white ones. Is there actually a clinical reason to do this? It is a reasonable question — and the answer is not always straightforward.
Silver fillings, technically called amalgam fillings, have been used for over 150 years. They are durable, affordable, and extremely effective at restoring damaged teeth. Millions of people have them.
Why Would a Dentist Recommend Replacement?There are legitimate clinical reasons to consider replacing amalgam. If a filling is cracked, worn, or showing signs of decay underneath it, replacement makes sense. Some patients also request replacement for cosmetic reasons — they dislike the dark appearance of silver fillings, particularly when visible near the front of the mouth.
There is also the mercury consideration. Amalgam contains mercury, bound to other metals. Some patients prefer to minimise their mercury exposure on principle, even though major health organisations including the NHS and WHO consider amalgam safe.
When Replacement May Not Be NecessaryIf your silver fillings are intact, properly sealed, and not causing any problems, there is no clinical urgency to replace them. Replacing a sound filling involves removing healthy tooth structure to accommodate the new restoration. Every replacement carries a small risk of creating new problems — cracks, sensitivity, or even the need for a root canal.
The age of your fillings matters too. Modern composite (white) fillings, while improved, may not last as long as well-maintained amalgam in large restorations.
The Real Reasons to DecideConsider replacement when: your filling is damaged or failing; you have a genuine allergy or sensitivity to amalgam; you are unhappy with the appearance and want a more natural look; or your dentist recommends it based on the condition of the tooth.
Do not feel pressured to replace simply because a new option exists. Your dentist should explain the specific benefits for your situation.
