Braces themselves — whether fixed or removable — do not directly damage gum tissue. However, they create surfaces where plaque can accumulate, and if oral hygiene isn't meticulous, the gums and the bone supporting your teeth can suffer during treatment.
How Braces Affect the Gums
Brackets and wires create small spaces where food and bacteria collect. If teeth are not cleaned thoroughly around each bracket, plaque builds up and causes gingivitis — red, swollen, bleeding gums. This is the most common gum problem during orthodontic treatment.
In more severe cases, lack of cleaning can lead to demineralisation — white marks on the enamel where acid from plaque has etched the surface. In the most extreme cases, if the bone is affected by prolonged inflammation, the gum can recede and the bone can shrink, leading to permanent gum recession.
Who Is Most at Risk
Patients who already have thin gum tissue or mild gum disease before braces are more vulnerable to gum recession during orthodontic treatment. Certain types of tooth movement — particularly moving teeth outward (labial tipping) — can put pressure on the gum margin and cause recession if the gum biotype is thin.
How to Protect Your Gums During Braces
The good news is that gum damage during orthodontics is almost entirely preventable with good hygiene. Use an interdental brush to clean around brackets. Use a water flosser or orthodontic floss threader to clean between teeth. Brush for at least two minutes after every meal. Avoid sugary and acidic foods and drinks that feed plaque. See a dental hygienist every three months during treatment.
